Once you have been approved by our department and UCLA David Geffen School of Medicine, you will need to make sure you have ample time to complete a few things, which are mandatory before beginning your ANES 350.03 rotation at Harbor-UCLA Medical Center.

You will be given detailed instructions to activate your name and information online “Los Angeles County Department of Health Services Information Sheet”. Once you have completed the information sheet, it will be reviewed and approved. You will then be given further instructions on how to proceed with the following.

 

Visit out Human Resources department located in Building F8 in order to complete your fingerprint/live scan. This MUST be done at least 2 weeks prior to the start date of your rotation. This cannot be done at any other type of facility besides our county facility. If you have had a live scan done at any of our neighboring county facilities such as Olive-View, LAC-USC, those will be acceptable as long as they are current. Please contact the student coordinator for verification. If you are unable to come to our facility to have a live scan done, please notify our course coordinator at least 2 months prior to the start of your rotation.

Upon completion of your live scan, you will also need clearance from employee health before beginning your rotation. Once you have been approved to do your ANES 350.03 rotation, the student coordinator will send you the employee health packet, which you will need to complete and send back to the student coordinator. Please contact our student coordinator for questions.

Some things to have in mind:

1. Please have your immunization records readily available
2. Current Proof of TB Skin Test 1 & 2
3. Current Respiratory Fit Testing (Kimberly-Clark N95 respirator only)
4. Current Flu Vaccine
5. There are additional health screening requirements. Please contact the student coordinator for a more detailed list of the requirements.
